Vancomycin, Trough
eDH Procedure Code(s)
LAB39
Collection Information
Trough level should be drawn 0-90 minutes (30 minutes optimal) before the dose.. Samples for antibiotic therapeutic drug monitoring are ideally drawn via peripheral venipuncture. When peripheral venipuncture is not possible refer to DH procedure ID 234 for guidance on appropriate collection site and steps to adequately flush the catheter/line. Subcutaneous ports (e.g. Mediports) should be avoided for sample collection due to a high risk of sample contamination.
