eDH Procedure Code(s)

LAB4376 Vancomycin Level, Random

Collection Information

This level is for the determination of the patient’s area-under-the- curve (AUC) value. Contact the inpatient pharmacy for interpretation.
Samples for antibiotic therapeutic drug monitoring are ideally drawn via peripheral venipuncture.
When peripheral venipuncture is not possible refer to DH procedure ID 234 for guidance on appropriate collection site and steps to adequately flush the catheter/line.
Subcutaneous ports (e.g. Mediports) should be avoided for sample collection due to a high risk of sample contamination.
