Botulism Toxin

Important Note

New Hampshire (NH) Bureau of Disease Control office must be notified of the requests for botulism toxin testing, 603-271-4496. Testing is performed by the CDC (Center for Disease Control and Prevention) on a case-by-case basis, 800-CDC-INFO (800-232-4636).

Specimen Collection Information

Acceptable specimens: Review with NHPHL/CDC.

Special paper requisition (CDC Form 50.34) must be completed by ordering provider and submitted to the laboratory with the specimen.

 

CDC accepts specimens from suspected botulism cases (no rule-out testing). CDC does not provide microbial identification or toxin testing on specimens not associated with clinical botulism cases.

Foodborne botulism: pre-antitoxin serum (without anti-coagulant), stool, enema (with sterile non-bacteriostatic water), food. Although not ideal, gastric contents may also be submitted.

Wound botulism: pre-antitoxin serum (without anti-coagulant), debrided tissue, swab from wounds, stool (only if foodborne is also suspected).

Infant botulism: stool, enema (with sterile non-bacteriostatic water), rectal swabs. Potential sources (honey or opened powdered formula) may also be submitted.

Unopened commercial products are accepted only with permission from FDA or USDA.

Specimen Transport Temperature

Refrigerate or Frozen- dependant on direction from NHPHL/CDC

Specimen Stability

Hold specimen until the physician contacts the CDC; refrigerate 2-8ºC.

Stool-  48 hours refrigerated or frozen

Specimen are sent directly to the CDC or via NH State lab per CDC direction/specifications.

Day(s) Performed

Specimens are sent to NHPHL/CDC Monday-Thursday by Microbiology Laboratory staff.
